Registration Options

1. Register online beginning Jan. 18, 2012 (credit card only)
2. Register by email: complete online form and email to newdentist@ada.org (credit card only)
3. Register by mail: complete online form, print and mail to: ADA, New Dentist Committee, 211 E. Chicago Ave., 20th Floor, Chicago, IL 60611-2678
4. Register by fax: complete online form and fax to 312.587.5101 (credit card only)

Please print or type all information and use one form per registrant. A check payable to the ADA or credit card authorization for registration fees must be received by the ADA at the time of registration. Registration forms without payment will not be processed.

On-Site Registration Hours
Friday, June 22, 2012: 7:30am–5:30pm

Please register early as courses fill quickly. Attendees will be registered for courses on a first-come, first-served basis. After June 12, 2012, you may register on-site at The Capital Hilton (an additional $50 registration fee will be applied). No cash payments will be accepted on-site—checks, credit card and money order payments will be accepted.

*Registration fees include:

• Full day of conference CE leadership programs on June 22
• Conference Opening Reception on June 22

**2012 membership dues must be paid to be eligible for ADA member rate. Eligibility for rates paid will be verified prior to the conference.

Guests of registered attendees are welcome to attend both the Opening Reception and the Saturday Night Event at The Park on Fourteenth. The Opening Reception fee for adult guests (13 years of age & older) is $15 per person. Children 12 years of age and under are free. The Saturday Night Event fee for adult guests (21 years of age & older) is $75 per person.

Cancellation Policy

All refund requests must be received in writing at the ADA New Dentist Committee office no later than June 12, 2012. Approved refunds or credits will be issued following the conference. Requests received after June 12, 2012 will not be eligible for a refund.
