Please print or type all information and use one form per registrant. A check payable to the ADA or credit card authorization for registration fees must be received by the ADA at the time of registration. Registration forms without payment will not be processed.

Please register early as courses fill quickly. Attendees will be registered for courses on a first-come, first-served basis. After June 12, 2012, you may register on-site at The Capital Hilton (an additional $50 registration fee will be applied). No cash payments will be accepted on-site—checks, credit card and money order payments will be accepted.

Guests of registered attendees are welcome to attend both the Opening Reception and the Saturday Night Event at The Park on Fourteenth. The Opening Reception fee for adult guests (13 years of age & older) is $15 per person. Children 12 years of age and under are free. The Saturday Night Event fee for adult guests (21 years of age & older) is $75 per person.

Cancellation Policy

All refund requests must be received in writing at the ADA New Dentist Committee office no later than June 12, 2012. Approved refunds or credits will be issued following the conference. Requests received after June 12, 2012 will not be eligible for a refund.
