- Download the ADA CERP Application for Recognition or contact the ADA CERP offices to have the application e-mailed or mailed to you by regular mail. For additional information, ADA members may call the 800 number and dial x2869 or 312-440-2869 or send an e-mail to: hendricksk@ada.org.
- Review the Standards and the application form to determine whether the provider can demonstrate compliance with the standards. Applications will be evaluated based on documented compliance with the eligibility requirements and the ADA CERP Recognition Standards and Procedures.
- Applications submitted by the deadline will be reviewed by the
ADA CERP Committee, a standing committee of the Council
on Dental Education and Licensure.

Next Deadline For Submission
January 9, 2009 is the next deadline for applications that will be reviewed at the Spring 2009 meeting of the ADA CERP Committee.
The decision of the Committee will be transmitted to the applicant in May 2009.
