In order to process your Give Kids A Smile sign-up form, you must accept the following terms outlined in this disclaimer.

If you do not accept the terms your sign-up will be canceled.

Thank you for acknowledging the following guidelines:

• Please do not exaggerate your program numbers; report them as accurately as possible. This allows the ADA to report program numbers to law- and policymakers. If you have not reported your numbers carefully, please return to the sign-up and do so.
• Completion of the Give Kids A Smile sign-up form does not constitute “placing an order” for professional or consumer product. You are simply requesting free product.
• Our sponsors will fulfill your request for free product if at all possible but cannot make any guarantees.
• Receiving product last year does not set a precedent; it may be that you do not receive it again this year.
