5. May I use the GKAS logo?
The Give Kids A Smile logo is readily available to dental societies for their use in promoting local events. If you are a dental society staffer, go to the Dental Society Resources Web site.
If you are serving as a chairperson of a local event and need the GKAS logo for a banner, handout or mailing that promotes the Give Kids A Smile event, please complete the request form below and e-mail it to gkas@ada.org. We will review and respond to your request as quickly as possible.

6. Whom do I contact if I want to participate with a GKAS program in my state?
Please contact your State Dental Society or e-mail gkas@ada.org.

7. Can I sign-up other dentists who want to participate on my GKAS team?
Yes, you can. On the Sign-up form, list yourself as the primary contact, then on the Event Information screen (Step 2 of 3) enter in the number of dentists who are participating. Add the dentists e-mail addresses on the Sign-up for E-communications form to ensure they receive all the latest reminders, news and updates about the program.

8. How do I withdraw from the program?
Withdraw your information on the Sign-up form by following the Delete Program instructions. If you do decide to withdraw, please remember to delete your program before the day of the event.

9. How can the public find out about programs in their state?
They can contact their local dental society and/or state dental society.

10. What are the sign-up dates for requesting products?
Product requests will be taken from October 1–November 14, 2008. Go to the Sponsors and Products page to learn more about products available.

11. When will I know the quantity and type of products I will receive (or when will the product be confirmed/denied)?

14. I missed the November 14, 2008 deadline for requesting free products. Can I still get them?
November 14 is the deadline for requesting free kits for Give Kids A Smile. This cutoff is necessary due to shipping requirements. We hope you will be among the first next year to submit your request for free products from our sponsors.

15. I'm in a foreign country; can I participate and receive free products?
You may participate, however you will not be able to receive products from our corporate sponsors.
