Please print or type all information. To register more than one attendee, please photocopy the registration form and submit together. A check payable to the ADA or credit card authorization for registration fees must be received by the ADA at the time of registration. Registration forms without payment will not be processed.

Please register early as courses fill quickly. Attendees will be registered for courses on a first-come, first-served basis. After June 17, 2010, you may register on-site at the Catamaran Resort Hotel and Spa (an additional $50 registration fee will be applied). No cash payments will be accepted on-site—checks, credit card and money order payments will be accepted.

Cancellation Policy
All cancellations must be received in writing at the ADA Committee on the New Dentist office no later than May 24, 2010. Approved refunds or credits will be issued following the conference. Cancellations received after May 24, 2010 will not be eligible for a refund.
