About your OAT PIN

Your OAT personal identification number (PIN) is a unique identifier you will use throughout the OAT process. If you have applied for the OAT or taken the test, you already have a PIN. 

When creating a new PIN, please note that the name you enter must exactly match the name on your government-issued identification (such as driver’s license or passport). On test day, the name on your ID must exactly match your OAT application or you will be denied admission and forfeit your testing fee.

Retrieve an existing PIN
If you have registered to take the OAT or have already taken the test, you already have a PIN. If you’ve forgotten it, you can retrieve it using the link below. You will need to enter:
  • Your full name as it appears on your government-issued ID
  • Date of birth
  • Email address

If you took the OAT under a different name (for example, a maiden name), do NOT register for a new PIN. Complete the PIN Request Form below and email it to the Department of Testing Services at PIN@ada.org for help.

If you know your PIN but have forgotten your password, use the link below to recover it.

If you have difficulty retrieving your PIN or password, complete the PIN Request Form below and email it to the Department of Testing Services at PIN@ada.org. We will get in touch with you to provide help.

Create a PIN 

If you are just getting started with the OAT, you will need to create a PIN before applying to take the test.

Here are tips to keep in mind:

  • The name you enter must exactly match the name on your government-issued identification (such as driver’s license or passport). On test day, the name on your ID must exactly match your OAT application or you will be denied admission and forfeit your testing fee.
  • We recommend that you use a personal email address instead of a school address, which may be obsolete when you graduate.
  • To ensure that emails from the Department of Testing Services (DTS) don’t end up in your spam folder, add ada.org to your approved senders list. (Please note that DTS is a shared service of the American Dental Association, working in cooperation with the Association of Schools and Colleges of Optometry.)

Changes or corrections to your information

If you’ve already created a PIN, but you need to change or correct the information you entered, do NOT register for a new PIN. Use the PIN Request Form below to request changes or corrections to your:

  • Name (for example, if your married or divorced name does not exactly match the ID you will present when taking the OAT)
  • Your birth date
  • Your email address

Email the completed form to the Department of Testing Services at PIN@ada.org. We will respond within 5 to 7 business days.
