Nonmembers of the House of Delegates may submit written testimony for reference committee hearings by downloading and completing the written testimony form. This form must be completed in full and sent to the appropriate Reference Committee email address below to be processed. Delegates and members who will be in attendance at the reference committee hearings and wish to provide testimony should provide their testimony during the hearings.

To be considered by the reference committee, comments must be received no later than 5:00 p.m. (Central Time) on Monday, October 2, 2023. Written testimony received by this deadline will be available for viewing by reference committee members and other members of the House of Delegates, and those with access to the House of Delegates (such as the ADA Executive Director, Council and Commission chairs, executive directors of state dental associations, and ADA staff). The contents of the written testimony forms will be posted in their entirety, including names, affiliations as well as any personally identifiable or confidential business information included on the form.
