United States of America Non-Commercial Licenses
SNODENT is owned, maintained and distributed by the American Dental Association (ADA). The SNODENT code set is available under license at no cost for non commercial use. The license agreement terms also permit licensees to use SNODENT in the development of non-commercial, academic, scholarly articles and presentations for publication. Instructions for Completing the SNODENT License Agreement Application
If the applicant is an organization, please fill the Address box with the Organization mailing address. The Application should be signed by the signing Authority.
If the applicant is an individual, please fill the Address box with the Individual mailing address.
For Applicants with an Electronic Signature
Complete one of the license forms below, and use the "Submit" button to apply for the SNODENT license electronically.
For Applicants WITHOUT an Electronic Signature
Complete one of the forms below, print out the form, sign it, and then scan the completed license form. Send the completed form as an attachment to email@example.com. The email should contain the following information:
Subject Line: SNODENT License Agreement
Body of Email: Name of the Applicant, email address where to send the SNODENT files, phone number, and a short description about the request.

To apply for a United States of America Commercial License, please complete and submit a U.S. Commercial SNODENT License Application (PDF).
IHTSDO Member Countries
International Licenses are available to IHTSDO member countries. A list of IHTSDO member countries can be found here.
For an International SNODENT License, complete and submit the IHTSDO Members SNODENT International License Application (PDF).
Non-IHTSDO Member Countries may apply for an international SNODENT license by completing and submitting the Non-IHTSDO Members SNODENT International License Application (PDF).
